Enrollment Requirements:
New students moving from another state to Nebraska are required to have a physical exam, vision exam and current immunizations. Ìý
All kindergarten and 7th grade students are required by State law to submit a current school physical, vision exam, and current immunizations.ÌýSee immunization requirements below:
KindergartenÌý
- 3 doses of DTaP, DTP, DT, or Td vaccine, one given on or after the 4th birthday
- 3 doses of Polio vaccine
- 2 doses of MMR vaccine, given on or after 12 months of age and separated by at least one month
- 3 doses of pediatric Hepatitis B vaccine or 2 doses of adolescent vaccine if student is 11-15 years of age
- 2 doses of varicella (chickenpox) Written documentation (including year) of varicella (chickenpox) disease from parent, guardian, or health care provider will be accepted
7th Grade
- 3 doses of DTaP, DTP, DT, or Td vaccine, one given on or after the 4th birthday
- 1 dose of Tdap (must contain Pertussis booster). This dose can be received any time after 10 or 11 years of age depending on which brand of vaccine is received.
- 3 doses of Polio vaccine
- 2 doses of MMR vaccine, given on or after 12 months of age and separated by at least one month
- 3 doses of pediatric Hepatitis B vaccine or 2 doses of adolescent vaccine if student is 11-15 years of age
- 2 doses of varicella (chickenpox) Written documentation (including year) of varicella (chickenpox) disease from parent, guardian, or health care provider will be accepted
Athletic Participation:
- Students participating in athletics must have submitted a current sports physical dated after May 1st.Ìý.
- The completed physical form, with vision exam must be returned by the first day of school, and immunizations must be up to date in order for your student to start school.Ìý
Annual School Health Screening:
- Nebraska Administrative Code 173 NAC 7, requires school districts to provide a health screening to enrolled students at specific grade levels.ÌýStudents in kindergarten, first, second, third, fourth, seventh, and tenth grades are screened for dental, vision (distance & near vision), and height and weight for the purpose of health monitoring for all students.

An ill student cannot learn effectively and is unable to participate in classes in a meaningful way.ÌýKeeping an ill student home prevents the spread of illness in the school community and allows the student an opportunity to rest and recover.
In case of illness or injury, a student will be sent home from school when the parent or legal guardian has been reached by telephone and appropriate arrangements are made for her or his transportation from school. If the school health office staff is unable to reach the parent or guardian, the authorized family emergency contacts will be notified.
If a student has a temperature of 100.4 degrees or higher, parents/guardians will be contacted and the student will be sent home as soon as proper arrangements can be made.
Students will be excluded from school for the following conditions: contagious or infectious disease, including chicken pox, measles and mumps; pink eye; suspicious rash; vomiting, diarrhea, seizure activity, skin eruptions or live head lice.
Students may not return to school until they are fever free for twenty-four (24) hours and/or bring a doctor’s note granting permission to return, or until all evidence of the condition is gone.

The following medications may be hand carried by a student at the given school levels:
Elementary:
- Inhaler with physician's order
- Epi-Pen with physician's order
Middle School:
- Inhaler with physician's order
- Epi-Pen with physician's order
- Cough drops with parent note turned into school's health room
High School:
- Inhaler with physician's order
- Epi-Pen with physician's order
- Non-prescription/OTC medication, including cough drops (no parent note needed)
- Students may transport prescription medication to the school's health room
